Abstract
Introduction: Complex defects in the middle and upper regions of the head pose a significant reconstructive challenge, particularly when they result from extensive tumor resections and are associated with multiple prior surgical interventions.
Among the available options for coverage, musculocutaneous flaps—both free and pedicled—allow for reconstruction in a single surgical stage. Currently, free flaps are considered the gold standard in the management of these complex defects. However, when their use is not feasible, regional flaps represent an excellent alternative. The most commonly used donor sites include flaps based on the supraclavicular artery, the pectoralis major musculocutaneous flap, and the trapezius musculocutaneous flap, whether from its middle or lower portion.
Materials and Methods: We present the clinical case of a patient with cutaneous squamous cell carcinoma located in the right retroauricular region, who underwent surgical resection with adequate oncologic margins in both surface and depth. In the same surgical procedure, the resulting defect was reconstructed using an island musculocutaneous flap based on the lower portion of the trapezius muscle.

Discussion and Conclusions: The technique described in this report represents a valid and effective option for the coverage of complex defects in the head and neck region. Its main advantages include versatility, adequate coverage provided by well-vascularized tissue, the possibility of performing reconstruction in a single surgical stage, and a low rate of associated complications.
